Clinical indication
Complementary to Mini-Cog in ≥65 when an informant (family member/caregiver) is available. More sensitive than patient-administered tests for early decline.
Instructions
Interview the INFORMANT (family member or caregiver, not the patient). For each question, indicate YES if there has been a change caused by cognitive problems, NO if no change, OVER SEVERAL YEARS.
