How it works

What it measures

The 10-year risk of a fatal or non-fatal cardiovascular event, in people aged 40-89 without prior CVD or diabetes.

Scoring

Combines 6 variables with the country's risk region → 10-year %. Risk thresholds depend on age.

Risk region · low/moderate/high/very high, by country

Interpretation

Low-moderate Low-to-moderate risk · <2.5% (<50y) · <5% (50-69) · <7.5% (≥70)

Lifestyle advice; generally no drug treatment.

High High risk · 2.5-7.5% · 5-10% · 7.5-15%

Consider treatment (statins, BP control) based on factors and preferences.

Very high Very high risk · ≥7.5% · ≥10% · ≥15%

Intensive risk-factor treatment recommended.

SCORE2 / SCORE2-OP (ESC 2021). Not applicable with established CVD, diabetes, moderate-severe CKD or familial hypercholesterolaemia (already high/very-high risk by definition).
