Fit note — statement of fitness for work (Med 3)
Decision support for the qualifying healthcare professional choosing between "not fit for work" and "may be fit for work" with reasonable adjustments (phased return, amended duties, altered hours, workplace adaptations), and the appropriate note duration and follow-up. Self-certification covers days 1–7; a Med 3 is required from day 8. Statutory Sick Pay runs up to 28 weeks (~196 days), after which form SSP1 routes the worker to Employment and Support Allowance (ESA).

Under the physician's clinical judgement
- This wizard is decision support only — it does not issue the Med 3 nor replace the clinical judgement of the qualifying healthcare professional.
- Occupational health advises on capability and reasonable adjustments; the employer decides what is reasonable to implement.
- A formal occupational health report requires the worker's informed consent (Access to Medical Reports Act 1988); the fit note itself is not such a report.
- No patient or worker data is stored — nothing is saved in the browser or on any server.
